Documents Needed to Process:

Child Passport.

Proof of identity for the parent or guardian

• Valid state photo ID or driver’s license of parents. (copy of front and back)
• Valid government issued photo ID of parents.

Proof of U.S. citizenship

• Certified copy of the child applicant’s birth certificate with the state’s seal. (Both parents must be on the certificate.)

Both parents listed on the birth certificate must be present at the post office, if one parent cannot be there, they need a notarized parental consent form DS-3053.

• Certificate of Citizenship or Naturalization.

If the child applicant has had a U.S. passport before, you must submit both the birth certificate and the expired passport.

Complete and print the passport application (DS-11) and Parent Consent Form

Fill out this form online and choose “create form” to view the completed application to be printed. Ignore the fees at this time. Please print 2 copies, but do not sign them yet.

• 1 Passport Size Picture: 2 Inches by 2 Inches on a white background
• 3 signed copies of the Letters of Authorization. Please print 3 and put an original signature on all 3 letters. This allows our company to act on the applicant’s behalf.
• Social Security Number of the child applicant.

If you do not provide the SSN, the IRS may impose a $500 penalty fee. If the child has just been born, write 000-00-0000

Copy of Travel Itinerary/Reservations

• A confirmed itinerary, OR a copy of your airline ticket, OR a letter from your company printed on your stationery addressed to the Washington Passport Agency introducing the applicant, stating the date and country of travel, requesting issuance of a new passport prior to your travel date.

Children Under 18 Years Old

If traveling with a child or children under the age of 18, then you must provide the following additional documents:

Photocopies of both parents’ driver's licenses and a copy of the information pages of their passports.

A photocopy of the minor's birth certificate showing both parents' full names, date of birth and nationality. Your birth certificate must be translated into English. You may also provide a notarized government document from your country of origin that shows parents' names, the minor's date of birth and nationality.

Only for persons of Indian origin and former Indian nationals: a birth certificate, a previous Indian passport, or a school leaving certificate. Please refer to the "Persons of Indian Origin Only" section for additional information.

Additionally, all minors aged five and above must sign their name in the signature box the first page of visa application under their photograph. Minors under age five should affix their thumb print in the blue or black ink in the same box. For all minor applicants both parents must sign their names on the second page and have their signatures duly authorized with a notary stamp and seal on the second page.

Both parents must also sign and complete the Parental Authorization Form for minors included in this kit. This form must be notarized.
